managing of members referred for service coordination. The Service

coordinate care between the members, providers and community resources. Responsibilities
also include the facilitation of a collaborative relationship among

Beacon’s
Intensive Case Management (ICM) program, through collaboration with
members, members’ guardian, representative and/or family member (s), state
agencies, schools, community providers and their treatment providers, is
designed to ensure the coordination of care, including assessment, case
management planning, discharge planning and mobilization of resources to
facilitate an effective outcome for cases with high risk clinical factors
and/or high utilization.  This
position includes field and telephonic contact.  

The case
manager works collaboratively with the member to advocate for and assist
with linkage to necessary supports and services, including community
resources, and to facilitate coordination with family and other involved
parties. An individualized care plan is developed with input from the
member and the member’s health care team. This individualized care plan
establishes and monitors short and long term goals and identifies resources
to assist in meeting the goals.
